Default Invoice format?

Learning from my lesson, I am now trying to draft out an common invoice to be used in general. However, I think that my invoice layout is very much too basic. At the moment, this is what I have on hand :-
1. To:
2. From:
3. Invoice Date :
4. Description Of Services
5. Hourly Rate
6. Total Estimated Work Hours
7. Total Project Cost
8. My company official stamp and authorized signature

What do you guys think? What else should be included?

Something smart to do is like I do, use an word document (like Microsoft Word or WordPerfect) and draw up your invoice, then convert it to a PDF using either CutePDF (which is free) or deskPDF.

I would help you out but am not even sure what the basis of your job is. I'd say project invoice, so I'll try and help you out.

9. Documentation (Always document what work you do, how long it takes you, etc.)

I personally think its just fine. I do agree that you should include a section for 'notes' so to speak where you can document what exactly you did for the project and how much time it took you. Even if you can't 'prove' it to your customer, they get a better idea of the actual work you did.

I'd say for your notes, put the times and date, I realized sometimes that makes your customer's/clients think, wow, this guy is SERIOUS about this, maybe I'll use him more often.

On an original contract, I'd include a project ID number, an estimate of when the work will be completed, and payment terms. For invoices during or after the project, I'd also include a description of work completed.
